Abstract
Polycystic ovary syndrome (PCOS) is a heterogeneous reproductive– metabolic disorder in which insulin resistance, hyperinsulinemia, androgen excess, altered adipose biology and ovarian dysfunction interact rather than operate as independent abnormalities. Objective: To critically synthesize the mechanistic bidirectionality of insulin resistance and hyperandrogenism and examine how randomized trials, real-world evidence and computational methods may refine disease stratification and treatment evaluation. Methods: A structured, non -systematic evidence synthesis prioritized the 2023 International Evidence-based PCOS Guideline, indexed mechanistic reviews, randomized-trial syntheses and regulatory documents concerning real-world evidence and artificial intelligence.
